What type of doctor or specialist can I speak with?

Members can talk to a doctor directly. Our doctors are licensed in internal medicine, family medicine and pediatrics. A doctor may also provide guidance on the type of specialist a member should see.

Can I get a prescription?

A prescription is not guaranteed. It is up to the doctor to recommend the best treatment. Doctor will submit your prescription directly to the pharmacy of the member’s choice.

Is my electronic health record kept private?

Health records are kept private and secure in order to protect members’ personal information. Only members can determine who can see the information in their records.

I don’t have a smartphone or computer, is there another way to talk with a doctor?

Yes, by dialing +1-317-893-3134 you can connect with care coordinator who can assist you with whatever it is you’re looking for, including enrollment, connecting with a doctor, or answering any technical support questions.

Is it possible to receive a doctor's note, or other medical forms?

Preciouslifes physicians can provide simple forms such as work/school excuses or return to work/school documents whenever clinically appropriate.

 

Please discuss your need for a work note with your Preciouslifes physician at the time of the appointment for swift processing.

 

We don’t complete specific forms such as FMLA, short term disability or travel restrictions. We recommend visiting an in person clinic if you need one completed.
